Third parties are required to upload a signed authorization or a consent form.

If paying by cheque, mail email confirmation and cheque together to the following address:

If paying by Debit, Credit, or Cash, attend in person at our Headquarters address above. Ensure you bring the printed email confirmation with you.

Once payment is received your request will be processed.
